A recent study sheds light on a chilling discovery: the epicenter of a multispecies superbug outbreak was traced back to unsuspecting culprits - the sinks of a pediatric ward. In 2017, the Toho University Omori Medical Center in Tokyo was rocked by a multispecies outbreak of carbapenemase-producing Enterobacterales (CPE) - a formidable superbug resistant to multiple antibiotics. The first case emerged in June 2016, when a one-year-old boy with cardiac disease fell victim to the deadly pathogen. Months later, in March 2017, a 15-year-old boy became the second casualty of the outbreak. What followed was a cascade of infections, spreading like wildfire among 19 pediatric patients.

Sink Surveillance:

As investigators delved deeper into the outbreak, a startling revelation emerged: the sinks harbored the elusive culprit. Nine sinks within the pediatric ward were found to be contaminated with CPE, serving as breeding grounds for the insidious bacteria. Six of these sinks were located in rooms housing CPE-positive patients, while the remaining three were situated in a nurse center, a waste room, and even an ice machine.

The Genetic Trail:

Genomic analysis uncovered a chilling truth - the superbug's resilience was not confined to a single species. Strains of Klebsiella variicola, Klebsiella quasipneumoniae, and Escherichia coli were identified, suggesting a complex network of bacterial transmission within the hospital environment. Even after replacing the sinks and thorough disinfection, the contamination persisted, hinting at a sophisticated resistance mechanism lurking within the hospital's plumbing.

Combatting the Threat:

Armed with knowledge, the hospital implemented stringent preventive measures to stem the tide of infection. Hand disinfection protocols, disposable cleaning tools, and strict prohibitions on mouth-washing with sink water were enforced. Every item exposed to sink water underwent rigorous disinfection and drying procedures. Finally, after months of tireless efforts, the outbreak was brought under control in October 2017.

Lessons Learned:

Reflecting on the ordeal, Associate Professor Sadako Yoshizawa emphasized the pivotal role of sinks and water-related areas in hospital wards as breeding grounds for antibiotic-resistant bacteria.
